Capital Campaign Project

For donations to the Rectory, we request you make donations via a check written to: 

"The Roman Catholic Charitable Trust" and a memo stating "St. Patrick Rectory"

Your check MUST be accompanied by this form:

Please print the form (2-sided) and turn it in with your check. 

For ONE TIME donations, please fill out your name, date, and the amount of the check on the front and print & sign your name on the back.

For RECURRING donations, please fill out your name on the front and print & sign your name on the back.

The Office will fill the date and amount of your check as received.
