Please print, review, and complete the following 5 pages:
Registration will take place at Our Lady of Mercy Office at 435 W 21st St.
Registration for the Spring 2026 Session is CLOSED.
Registration dates for Fall 2026 will be released after Easter.
Please anticipate spending some time there as this registration will also act as a sort of intake interview.
There will also likely be a line. Be aware that we do not have a specified cap on acceptance, but we MUST have all required paperwork in order to accept your registration.
A few notes that can be found in the packet, but are important to note:
Registration of $35 must be paid via cash or check (we cannot accept card).
County Birth Certificate.
A NEWLY ORDERED Baptismal Certificate with all Notations is required to be turned into the leadership team ASAP!
This is a copy of your Baptismal Certificate from your church of Baptism with notations listing all other Sacraments received. To obtain, you must reach out to the church where you were Baptized and ask for a "Baptismal Certificate with Notations".
If you were Baptized at St. Patrick's/Our Lady of Mercy Church, we can provide a written copy to you. Please reach out to the office during open office hours.
Start thinking about your Sponsor
This person will walk this faith journey with you! This needs to be someone who is a fully initiated and practicing Catholic. If they are married, they must be married in the Church. The Sponsor form is included in your packet above. Please review and ask any question you may have at registration. You will also need to provide your Sponsor's Sacrament Certificates and a Letter of Permission from their home parish if they do not regularly attend St. Patrick's.
If you are told at the registration that you need to set an appointment with Fr. Robert for anything pertaining to marriage and relationships, please be sure that appointment is set as soon as possible by calling our main office at 209-383-3924.
